This whistleblowing channel provides a dedicated avenue to report any incidents of potential abuse of grant, fraud or misconduct that involve recipients of the NEA-administered Coffeeshop Toilet Renovation Grant (CTRG) and Coffeeshop Toilet Deep Cleaning Grant (CTCG).
Information to be provided
In reporting any fraudulent or unethical activity, you shall, to the best of your ability, ensure that the information you provide is accurate and complete.
Your report should provide the following information where possible:
Description of the fraudulent or unethical activity;
Name(s) of person(s)/institution(s)/company(ies) allegedly involved;
Name of Grant [including Grant Reference ID], where applicable;
Date, time and location of incident (including multiple occurrences);
How and when you come to know of the activity;
Estimated value of any money or asset involved;
Names and contact details of any witnesses;
Supporting documents, evidence (e.g. photos, videos, documents), if any; and
Any other information that may substantiate the concern.
We encourage whistle-blowers to leave their name and contact information in case further information or clarifications are required. The effectiveness of our investigations depends on the sufficiency and quality of the information provided.
